The Northside Hospital Forsyth Auxialiary is committed to the professional develpment of healthcare students and to providing assistance with those pathways of learning. Therefore, the scholarship selection committee is offering the enclosed award opportunities:

2011-2012 scholarship applications are available to students engaged in or planning a career in a health related field. Each scholarship has individual guidelines and requirements for submission. Please note; only one (1) completed application is necessary for the two local scholarships.
BE SURE TO FOLLOW ALL INSTRUCTIONS REGARDING THE SCHOLARSHIP AWARDS BEFORE COMPLETION AND RETURN TO THE SCHOLARSHIP COMMITTEE.
Please be certain that ALL required information is included with each application that is submitted: specifically reference letters, narrative profile, official (unopened) transcripts. Completed application forms and additional information must be received no later than the dates indicated above.
The selection committee requires a personal interview with each applicant. With this requirement in mind, students who will be at home during the winter holiday should consider early completion and submission of applications. This may expedite the required assessment and interview process for each applicant. The NSFH Auxiliary Committee will review the submitted applications as soon as possible after receipt of the required formalities.
