Dear Prospective Volunteer,
We want to thank you for expressing an interest in volunteering at Northside Hospital-Forsyth. We welcome people who are available and willing to make a commitment to give of their time in service to our community. All Northside Hospital-Forsyth Volunteers are required to be members of the Auxiliary, attend an initial orientation, purchase a volunteer uniform to wear while on duty, have an annual TB skin test and pay annual dues. In addition, you are required to work your once a week four-hour commitment in your assigned service.
You may download and complete the Volunteer Application and Background Authorization forms. Please print these forms making sure you sign and date both pages. Then mail or deliver the application and background release forms to the Northside Hospital–Forsyth Auxiliary office located at 1200 Northside Forsyth Drive, Cumming, GA 30041. Also you will need to have an interview appointment with Gloria Wyatt, Coordinator of Volunteer Services or one of the officers of the Auxiliary. Someone will call you to set up a time convenient for both of you to have this interview. If you need to reach the Auxiliary office, the phone number is 770.844.3431. We will need your completed application before this interview.
After orientation each volunteer will be assigned to two four hours shifts at the Hospital Transport Team Desk. This is a good way to acquaint you with the hospital’s many departments and a chance to meet other people. During this time the Vice – President of Orientation and Training will discuss possible volunteer assignments and contact you either by phone or in person.
Again, thank you for your interest and we look forward to welcoming you to the Northside Hospital-Forsyth Auxiliary. Orientations are scheduled for the third Wednesday of each month and last approximately four hours followed by lunch in the hospital cafeteria.
