Instructions and Guidelines:
1. Review and be sure all guidelines on the scholarship agreement are followed.
2. GPA must be at least 3.0.
3. Applicant must be already accepted into a medical related program. This does not include pre-med or pre-nursing.
4. Be sure OFFICIAL transcript is included (not a faxed copy or a photocopy).
5. All information will remain confidential.
6. Check that all four (4) pages of the application have been completed, that page 3 and page 4 have been signed by the applicant and that the applicant's signature has been witnessed. Read the instructions on page 4 carefully.
7. Return in a sealed envelope to Northside Hospital Forsyth no later than
February 1, 2012.
Please take to the Information Desk in the main lobby of the hospital or to the Resource Center (Suite 110, 1400 Building, Northside Hospital Forsyth). Include: "Attention Scholarship Committee" on the envelope.
