Instructions and Guidelines:
These scholarships are awarded to students currently pursuing a health related detree at an accredited college, university or health related technical school.
To qualify the applicant must:
1. Be a full time or a regular part time employee of the Northside Hospital-Forsyth.
2. Be a a family member of a full time or a regular part time employee of Northside Hospital-Forsyth.
3. Be a current Northside Hospital-Forsyth Auxilian who has served at least 100 volunteer hours.
4. Be a family member of a Northside Hospital-Forsyth Auxilian who has served at least 100 volunteer hours.
5. Have at least a 3.0 GPA in a health related course of study.
6. Provide an official, sealed transcript (not a photocopy or faxed copy) from an accredited institution.
7. Provide two (2) letters of recommendation from a teacher, counselor, work supervisor, or clergy, with current date, appropriate letterhead and in a seled envelope.
8. Submit a profile of yourself noting experience that is pertinent to your career goals. Include extra curricular activities, sports, volunteer, part-time or full time employment.
9. Be available for a personal interview with the scholarship committee.
The application forms are due to be returned to the Northside Hospital-Forsyth Auxiliary Committee no later than
March 30, 2012.
Return the completed applications in a sealed envelpe to the Information Desk or the Resource Center. Att'n: Scholarship Committee - Suite 110, 1400 Building, Northside Hospital Forsyth. You will receive an acknowledgement of receipt of the applications from the scholarship committee.
